If accepted for this trip, I will participate voluntarily and of my own free will.
I will not hold team leaders, Michael E. Winslett, Baseball 4 Christ USA, Inc. or sponsoring mission/missionaries responsible for any accident, injury, illness or other personal loss that might result from this trip.
I authorize team leaders, as my agents, to consent to any emergency treatment that is necessary in the case of accident or illness, which is deemed advisable.
I will submit to team leadership and maintain a cooperative spirit in all activities.
To the best of my ability, I will participate in the trip and preparation and evaluation sessions.

RULES

Follow leadership Instructions at all times.
No inappropriate social media that may be disrespectful to Baseball 4 Christ USA, Inc.
After curfew/lights out -no one outside their assigned rooms.
No student may go anywhere without adult supervision, adult permission or without an interpreter.
No alcohol, foul language or behavior that reflects negatively against Baseball 4 Christ USA, Inc.
No tobacco of any type.
Maintain a cooperative spirit In all mission activities.
To the best of your ability, participate In all mission activities.
Swim Suit -No underwear exposed.

Failure to obey these guidelines may result in the student being removed from the team. Items noted In Red will result in immediate removal from Baseball 4 Christ USA, Inc. and returned back to the U.S. at student's expense.
